Summary
Both drugs prolong the QT interval, and fluconazole may also reduce dofetilide elimination, raising its levels. This markedly increases the risk of torsades de pointes and fatal ventricular arrhythmia.
What this interaction means in plain words
Dofetilide and Fluconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Dofetilide belongs to Cardiovascular; Fluconazole belongs to Anti Fungals.
Why it happens
Additive QT prolongation, with possible fluconazole-related increase in dofetilide exposure via CYP3A4 and renal transport inhibition.
What to do
Avoid the combination; use a non-QT-prolonging antifungal. If systemic azole therapy is essential, consider an alternative antiarrhythmic.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
