Summary
Mavacamten is cleared mainly by CYP2C19, with a contribution from CYP3A4, and fluconazole inhibits both enzymes. Higher mavacamten exposure can excessively reduce ejection fraction and cause heart failure from systolic dysfunction. Labeling contraindicates moderate to strong CYP2C19 inhibitors.
What this interaction means in plain words
Fluconazole and Mavacamten are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Fluconazole belongs to Anti Fungals; Mavacamten belongs to Cardiovascular.
Why it happens
Fluconazole inhibits CYP2C19 and CYP3A4, reducing mavacamten clearance.
What to do
Do not combine them. If fluconazole is needed, interrupt mavacamten per labeling and monitor LVEF.
What to ask your pharmacist

- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
