Summary
Fluconazole can raise exposure to both components of the inhaled combination, mainly through CYP3A4 inhibition. Higher vilanterol exposure may amplify beta-agonist cardiovascular effects such as tachycardia, palpitations and QT prolongation, and higher fluticasone furoate exposure may increase systemic corticosteroid effects. Fluconazole itself can prolong the QT interval.
What this interaction means in plain words
Fluconazole and Vilanterol / fluticasone furoate inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Fluconazole inhibits CYP3A4, reducing clearance of vilanterol and fluticasone furoate, with possible additive QT prolongation.
What to do
Monitor heart rate, ECG and potassium in patients with cardiac risk factors, and watch for systemic corticosteroid effects during prolonged use. Consider an alternative antifungal or bronchodilator if QT risk is high.
