Summary
Fluoroquinolones carry a risk of tendinitis and tendon rupture that is increased by concurrent corticosteroids. Fludrocortisone is mainly mineralocorticoid, so its contribution is probably smaller, but the risk is still considered additive. Hypokalemia from fludrocortisone could also add to QT concerns.
What this interaction means in plain words
Fludrocortisone and Gemifloxacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fludrocortisone belongs to Endocrine; Gemifloxacin belongs to Anti Bacterial.
Why it happens
Additive tendon toxicity with corticosteroid exposure; mechanism involves impaired tenocyte function and collagen turnover.
What to do
Counsel the patient to stop the antibiotic and report tendon pain or swelling at once. Monitor potassium, and consider an alternative antibiotic in older patients or those on other corticosteroids.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
