Summary

Combining a corticosteroid with an NSAID increases the risk of gastrointestinal irritation, ulceration and bleeding. Both drugs can also cause fluid retention and raise blood pressure, which may be relevant given fludrocortisone's mineralocorticoid action.

What this interaction means in plain words

Fludrocortisone and Lornoxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fludrocortisone belongs to Endocrine; Lornoxicam belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive gastric mucosal injury (NSAID prostaglandin inhibition plus corticosteroid effects) and additive sodium and fluid retention.

What to do

Use the lowest effective NSAID dose for the shortest time, consider gastroprotection with a proton pump inhibitor, and monitor blood pressure, edema and GI bleeding signs.

What to ask your pharmacist

  • Should I take them at different times of day?
