Summary

Histrelin, a GnRH agonist, can prolong the QT interval through androgen deprivation, and fluoxetine also has QT-prolonging potential. Taken together they may add to the risk of torsades de pointes, especially in patients with other risk factors.

What this interaction means in plain words

Fluoxetine and Histrelin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fluoxetine belongs to Central Nervous System; Histrelin belongs to Musculoskeletal.

Evidence level: Low

Why it happens

Additive QT prolongation through androgen deprivation (histrelin) and cardiac ion channel effects (fluoxetine).

What to do

Obtain a baseline ECG and correct potassium and magnesium before starting. Monitor the QTc periodically and consider an alternative antidepressant if the QTc becomes markedly prolonged.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
