Summary

Quizartinib markedly prolongs the QT interval and can cause torsades de pointes, and fluoxetine adds further QT effect. Fluoxetine is also a CYP3A inhibitor of limited strength, which may modestly raise quizartinib exposure.

What this interaction means in plain words

Fluoxetine and Quizartinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Fluoxetine belongs to Central Nervous System; Quizartinib belongs to Oncology.

Evidence level: Moderate

Why it happens

Additive QT prolongation; fluoxetine may also weakly inhibit CYP3A-mediated quizartinib metabolism.

What to do

Avoid the combination where possible and consider an alternative antidepressant. If combined, monitor ECG and electrolytes closely and follow the quizartinib labeling for dose reduction with CYP3A inhibitors.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
