Summary
Ribociclib is a moderate CYP3A4 inhibitor and may raise systemic fluticasone exposure, although inhaled fluticasone has very low baseline bioavailability. Ribociclib also prolongs the QT interval, and salmeterol can add to this effect at higher exposures.
What this interaction means in plain words
Fluticasone-salmeterol and Ribociclib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluticasone-salmeterol belongs to Respiratory; Ribociclib belongs to Oncology.
Why it happens
Ribociclib inhibits CYP3A4, reducing metabolism of fluticasone and salmeterol, with additive QT-prolonging potential.
What to do
Monitor for corticosteroid systemic effects such as adrenal suppression with prolonged use, and review ECG and electrolytes as per ribociclib monitoring. Report palpitations or syncope promptly.
What to ask your pharmacist
