Summary
Mifepristone, particularly at the higher doses used for Cushing's syndrome, inhibits CYP3A4 and prolongs QT. This can raise fluticasone furoate and vilanterol exposure, increasing corticosteroid and cardiovascular effects.
What this interaction means in plain words
Fluticasone-vilanterol and Mifepristone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluticasone-vilanterol belongs to Respiratory; Mifepristone belongs to Endocrine.
Why it happens
CYP3A4 inhibition by mifepristone increases inhaled drug exposure, with additive QTc prolongation.
What to do
Avoid the combination where possible or use an alternative inhaler not dependent on CYP3A4. If combined, monitor ECG, potassium and signs of systemic corticosteroid excess.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
