Summary
Aprepitant, the active form of fosaprepitant, is a moderate CYP3A4 inhibitor and could modestly increase irinotecan exposure. Studies of aprepitant with irinotecan-based chemotherapy have not shown clinically important changes in irinotecan or SN-38 exposure. The combination is commonly used for antiemetic prophylaxis.
What this interaction means in plain words
Fosaprepitant and Irinotecan have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Fosaprepitant belongs to Gastrointestinal Tract; Irinotecan belongs to Oncology.
Why it happens
Aprepitant weakly to moderately inhibits CYP3A4, which could slightly reduce irinotecan oxidative clearance.
What to do
No dose change is usually needed; monitor for diarrhea and neutropenia as per routine irinotecan care.
