Summary
Isoproterenol drives potassium into cells through beta-2 stimulation, and hydrochlorothiazide promotes renal potassium loss. Together they can produce clinically relevant hypokalemia, which raises the risk of arrhythmia. The ACE inhibitor component only partly offsets this.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Isoproterenol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Isoproterenol belongs to Respiratory.
Why it happens
Additive lowering of serum potassium via beta-2 mediated cellular uptake plus thiazide-induced renal potassium wasting.
What to do
Monitor serum potassium and ECG when isoproterenol is given to patients on this combination. Replace potassium and correct magnesium as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
