Sulindac may blunt the antihypertensive effect of fosinopril and hydrochlorothiazide. Its renal prostaglandin-sparing effect is less certain than other NSAIDs, but renal impairment and hyperkalemia can still occur, especially in at-risk patients.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Sulindac belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis can reduce renal perfusion and sodium excretion, opposing ACE inhibitor and thiazide effects.
What to do
Prefer a non-NSAID analgesic. If used, choose the lowest effective dose and monitor blood pressure, creatinine and potassium.
