Summary

Fosphenytoin is converted to phenytoin, a broad enzyme inducer that can lower vortioxetine concentrations. This may lead to loss of antidepressant effect and relapse of depressive symptoms.

What this interaction means in plain words

Fosphenytoin and Vortioxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fosphenytoin belongs to Central Nervous System; Vortioxetine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Phenytoin induces CYP enzymes (including CYP3A4 and CYP2C9) involved in vortioxetine metabolism, increasing its clearance.

What to do

Monitor clinical response to vortioxetine and consider increasing the dose, up to a maximum of triple the usual dose, when a strong inducer is used long term. Reduce vortioxetine back to the original dose within about 14 days of stopping the inducer.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
