Summary
Lofexidine lowers blood pressure and heart rate, and furosemide adds blood pressure reduction through volume depletion. Together they can cause marked hypotension, dizziness and syncope. Furosemide-induced hypokalemia or hypomagnesemia may also add to lofexidine's QT risk.
What this interaction means in plain words
Furosemide and Lofexidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Furosemide belongs to Cardiovascular; Lofexidine belongs to Central Nervous System.
Why it happens
Additive hypotensive effects of central alpha-2 agonism and loop diuretic volume depletion, with electrolyte loss adding to QT risk.
What to do
Monitor blood pressure, pulse and electrolytes, especially during dose changes. Reduce the dose of either drug if symptomatic hypotension or bradycardia develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
