furosemide + methyldopa

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: High

Additive antihypertensive effects increase risk of hypotension/orthostasis, especially with diuretic-induced volume depletion.

Mechanism of action

Pharmacodynamic additivity: loop diuresis/natriuresis (furosemide) plus central alpha-2 agonism (methyldopa) lowers BP and may potentiate orthostatic hypotension.

Clinical management

Monitor BP (including orthostatics) and renal function/electrolytes; start low and titrate; consider dose reduction if symptomatic hypotension occurs; ensure adequate volume status.
