Summary
Adding an ACE inhibitor to a loop diuretic, especially in volume-depleted patients, can cause marked first-dose hypotension and a decline in renal function. The risk is greatest at initiation or after a dose increase, or with sodium depletion or renal artery stenosis.
What this interaction means in plain words
Furosemide and Moexipril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Furosemide belongs to Cardiovascular; Moexipril belongs to Cardiovascular.
Why it happens
Volume depletion from diuresis combined with ACE inhibition reduces angiotensin II-mediated efferent arteriolar tone, causing additive hypotension and reduced glomerular filtration.
What to do
Consider temporarily reducing or holding furosemide before starting moexipril, and start with a low dose. Monitor blood pressure, serum creatinine and potassium within the first one to two weeks.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
