Gabapentin + Pregabalin

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Pharmacodynamic duplication of gabapentinoids can lead to additive CNS depression (sedation, dizziness, ataxia, cognitive impairment) and may increase risk of respiratory depression and peripheral edema.

Mechanism of action

Pharmacodynamic: both bind α2δ subunit of voltage-gated calcium channels leading to additive CNS/respiratory depressant effects; no significant pharmacokinetic interaction (both renally eliminated unchanged).

Clinical management

Avoid therapeutic duplication when possible. If transitioning, cross-taper rather than combine. If concomitant use is unavoidable, use lowest effective doses, adjust for renal function, and monitor for excessive sedation, respiratory depression, falls, and edema.
