Summary
Gentamicin is nephrotoxic, and volume depletion from the thiazide plus ARB-mediated reduction in glomerular pressure can increase the risk of acute kidney injury. Diuretics may also add to aminoglycoside-related electrolyte disturbances such as hypokalemia and hypomagnesemia.
What this interaction means in plain words
Gentamicin and Hydrochlorothiazide-telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gentamicin belongs to Anti Bacterial; Hydrochlorothiazide-telmisartan belongs to Cardiovascular.
Why it happens
Reduced renal perfusion and volume depletion increase aminoglycoside accumulation and tubular toxicity, with additive renal electrolyte wasting.
What to do
Monitor creatinine, gentamicin levels, potassium and magnesium, and keep the patient well hydrated. Consider temporarily withholding the ARB-diuretic product during acute illness.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
