Summary
Thiazide-induced potassium and magnesium loss can add to the electrolyte wasting caused by aminoglycosides. Volume depletion from the diuretic and the effect of valsartan on renal hemodynamics may also raise the risk of gentamicin nephrotoxicity and ototoxicity.
What this interaction means in plain words
Gentamicin and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gentamicin belongs to Anti Bacterial; Hydrochlorothiazide-valsartan belongs to Cardiovascular.
Why it happens
Additive renal electrolyte wasting, and volume depletion with reduced glomerular perfusion that favors aminoglycoside accumulation and tubular toxicity.
What to do
Monitor serum creatinine, potassium and magnesium during gentamicin therapy and monitor gentamicin levels. Replace electrolytes and consider holding the diuretic and valsartan if renal function worsens or the patient becomes volume depleted.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Gentamicin with Hydrochlorothiazide-valsartan?
MediHelp rates the interaction as moderate. Gentamicin and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
How serious is the interaction between Gentamicin and Hydrochlorothiazide-valsartan?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor serum creatinine, potassium and magnesium during gentamicin therapy and monitor gentamicin levels. Replace electrolytes and consider holding the diuretic and valsartan if renal function worsens or the patient becomes volume depleted.
What other medicines interact with Hydrochlorothiazide-valsartan?
MediHelp also lists interactions of Hydrochlorothiazide-valsartan with, for example, Aliskiren, Dofetilide, Sparsentan. The full list is on the Hydrochlorothiazide-valsartan page.
