Summary

NSAIDs can reduce renal function and glomerular filtration, which can decrease gentamicin clearance and raise serum levels. This increases the risk of nephrotoxicity and ototoxicity, especially in neonates, the elderly or volume-depleted patients.

What this interaction means in plain words

Gentamicin and Ibuprofen can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Gentamicin belongs to Anti Bacterial; Ibuprofen belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

NSAID-induced reduction of renal prostaglandin-dependent perfusion lowers aminoglycoside clearance, and both drugs are nephrotoxic.

What to do

Avoid combining where possible; if needed, monitor serum creatinine and gentamicin trough levels and adjust the aminoglycoside dose accordingly.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
