Summary
Both drugs can prolong the QTc interval, so using them together may add to the risk of torsades de pointes and other serious ventricular arrhythmias. The risk is higher with electrolyte disturbances, bradycardia or underlying heart disease.
What this interaction means in plain words
Givinostat and Paliperidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Givinostat belongs to Miscellaneous; Paliperidone belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic QT prolongation through cardiac repolarization effects (hERG potassium channel blockade).
What to do
Prefer an alternative to one of the agents. If both are needed, obtain a baseline ECG and repeat during co-treatment, correct potassium and magnesium, and withhold if QTc exceeds 500 ms or rises more than 60 ms from baseline.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
