Summary
Fluoroquinolones can cause both severe hypoglycemia and hyperglycemia, particularly in diabetic patients taking sulfonylureas. Moxifloxacin carries a lower risk than some other quinolones, but serious episodes have been reported, especially in older patients and those with renal impairment.
What this interaction means in plain words
Gliclazide and Moxifloxacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gliclazide belongs to Endocrine; Moxifloxacin belongs to Anti Bacterial.
Why it happens
Quinolones block pancreatic ATP-sensitive potassium channels, increasing insulin release and adding to the sulfonylurea effect.
What to do
Monitor blood glucose closely during and shortly after the antibiotic course, and teach the patient to recognize both hypo- and hyperglycemia. Stop the quinolone and choose another antibiotic if significant dysglycemia occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
