Summary

NSAIDs may enhance the glucose-lowering effect of sulfonylureas and occasionally cause hypoglycemia. The effect is inconsistent and more likely with highly protein-bound NSAIDs, in the elderly, or with renal impairment.

What this interaction means in plain words

Glimepiride and Oxaprozin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Glimepiride belongs to Endocrine; Oxaprozin belongs to Musculoskeletal.

Evidence level: Low

Why it happens

Oxaprozin is highly protein bound and may displace sulfonylureas from albumin, and NSAID effects on renal prostaglandins may reduce sulfonylurea clearance; the mechanism is not fully established.

What to do

Monitor blood glucose when starting or stopping oxaprozin and teach patients to recognize hypoglycemia. Reduce the glimepiride dose if glucose falls.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
