Summary
Glofitamab causes cytokine release that may transiently suppress CYP450 enzymes and increase exposure to CYP3A4 substrates. Higher pimozide levels increase the risk of QT prolongation and serious ventricular arrhythmias.
What this interaction means in plain words
Glofitamab and Pimozide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Glofitamab belongs to Oncology; Pimozide belongs to Central Nervous System.
Why it happens
Cytokine-mediated suppression of CYP3A4 activity during cytokine release reduces pimozide clearance.
What to do
Monitor ECG/QTc and for pimozide toxicity during the initial dosing period and any cytokine release syndrome. Consider reducing or holding pimozide, or using an alternative.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
