Summary
Glofitamab induces cytokine release that can transiently suppress CYP enzyme activity, which may raise levels of drugs metabolized by CYP1A2, such as clomipramine. The effect is greatest after the first doses and during cytokine release syndrome.
What this interaction means in plain words
Clomipramine and Glofitamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Clomipramine belongs to Central Nervous System; Glofitamab belongs to Oncology.
Why it happens
Cytokine-mediated suppression of CYP enzymes, including CYP1A2, reduces clomipramine metabolism.
What to do
Monitor for tricyclic toxicity such as anticholinergic effects, sedation and arrhythmia during the initial treatment cycles. Reduce the clomipramine dose if signs of toxicity appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
