Summary

Glofitamab triggers cytokine release, and raised cytokines such as IL-6 can suppress CYP450 enzymes, including CYP1A2. Clozapine levels may therefore rise, particularly around the first doses or during cytokine release syndrome, increasing the risk of sedation, seizures, hypotension and other clozapine toxicity.

What this interaction means in plain words

Clozapine and Glofitamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Clozapine belongs to Central Nervous System; Glofitamab belongs to Oncology.

Evidence level: Low

Why it happens

Cytokine-mediated transient suppression of CYP1A2 reduces clozapine clearance.

What to do

Monitor for clozapine toxicity (sedation, hypotension, seizures) during the first cycle and any cytokine release syndrome episodes. Consider checking clozapine concentrations and reducing the dose if toxicity appears.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
