Summary
Guanfacine, a central alpha-2 agonist, reduces sympathetic outflow and may blunt the adrenergic warning signs of hypoglycemia such as tremor and palpitations. A patient taking glyburide may therefore fail to recognize falling blood glucose early. Sweating is typically preserved.
What this interaction means in plain words
Glyburide and Guanfacine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Glyburide belongs to Endocrine; Guanfacine belongs to Cardiovascular.
Why it happens
Central sympatholytic action reduces the catecholamine-mediated symptoms of hypoglycemia.
What to do
Advise patients to monitor blood glucose routinely rather than rely on symptoms. Teach them that sweating and hunger may be the main warning signs.
What to ask your pharmacist
- Is there anything I should notice while taking both?
- Do I need to tell my doctor at the next visit?
