Summary
Chronic inflammation can suppress CYP450 enzyme activity through raised cytokines, and effective IL-23 blockade may restore normal enzyme activity. This could modestly change levels of CYP substrates such as thioridazine. A clinically meaningful effect has not been shown for guselkumab.
What this interaction means in plain words
Guselkumab and Thioridazine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Guselkumab belongs to Miscellaneous; Thioridazine belongs to Central Nervous System.
Why it happens
Theoretical normalization of cytokine-suppressed CYP450 activity when inflammation is controlled.
What to do
No dose change is routinely needed. Monitor clinical response and ECG if thioridazine effect appears to change after guselkumab is started or stopped.
