Haloperidol and morphine together can cause additive sedation, hypotension and respiratory depression. Haloperidol inhibits CYP2D6, but this has little impact on morphine, which is cleared mainly by glucuronidation.

What this interaction means in plain words

Haloperidol and Morphine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Haloperidol belongs to Central Nervous System; Morphine belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive CNS depression; minor CYP2D6 inhibition by haloperidol is of limited relevance to morphine.

What to do

Monitor sedation, respiratory rate and blood pressure, particularly when doses are increased. Combination is commonly used in palliative care with titrated low doses.

What to ask your pharmacist
