Summary

Heparin suppresses adrenal aldosterone synthesis, and irbesartan also reduces aldosterone effect, so together they can cause hyperkalemia. The risk is greater in diabetes, renal impairment, or prolonged heparin use. The thiazide component partly offsets potassium retention.

What this interaction means in plain words

Heparin and Hydrochlorothiazide-irbesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Heparin belongs to Cardiovascular; Hydrochlorothiazide-irbesartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive suppression of the renin-angiotensin-aldosterone axis leading to reduced renal potassium excretion.

What to do

Monitor serum potassium and renal function, especially if heparin is given for more than a few days or in patients with kidney disease or diabetes. Treat hyperkalemia or reconsider therapy if potassium rises.
