Summary
Thiazide diuretics can raise blood glucose and partly offset the glucose-lowering effect of insulin. Mealtime insulin requirements may rise, particularly with higher thiazide doses.
What this interaction means in plain words
Hydrochlorothiazide and Insulin lispro have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Hydrochlorothiazide belongs to Cardiovascular; Insulin lispro belongs to Endocrine.
Why it happens
Thiazide-induced hypokalemia and reduced insulin secretion and peripheral insulin sensitivity oppose the action of insulin (pharmacodynamic antagonism).
What to do
Monitor blood glucose when the hydrochlorothiazide is started, stopped or changed and adjust the insulin dose as needed. Check serum potassium periodically.
