Summary

Norepinephrine given intravenously may reduce potassium through beta-2 effects, and combined with thiazide kaliuresis could worsen hypokalemia. Thiazides may also slightly reduce vascular responsiveness to pressors.

What this interaction means in plain words

Hydrochlorothiazide and Norepinephrine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive potassium lowering via renal loss and beta-adrenergic intracellular shift; thiazide may blunt pressor response.

What to do

Monitor serum potassium and blood pressure during norepinephrine infusion and replace potassium as needed.

What to ask your pharmacist
