Summary

Trilostane inhibits adrenal steroid synthesis including aldosterone, which reduces potassium loss and may enhance the natriuretic effect. Combined with hydrochlorothiazide, this can cause hyperkalemia, hyponatremia or hypovolemia in susceptible patients.

What this interaction means in plain words

Hydrochlorothiazide and Trilostane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Inhibition of 3-beta-hydroxysteroid dehydrogenase lowers aldosterone, opposing thiazide-induced potassium loss and adding to sodium loss.

What to do

Monitor serum sodium, potassium, blood pressure and renal function when combining the drugs. Adjust doses or stop the thiazide if electrolyte abnormalities develop.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
