Summary
Both thiazides and corticosteroids promote potassium loss, so combined use can lead to hypokalemia. This is more likely with prolonged or high-dose steroid therapy and may provoke arrhythmias or weakness.
What this interaction means in plain words
Hydrochlorothiazide-irbesartan and Hydrocortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-irbesartan belongs to Cardiovascular; Hydrocortisone belongs to Endocrine.
Why it happens
Additive renal potassium wasting from thiazide-induced distal sodium delivery and glucocorticoid mineralocorticoid activity.
What to do
Monitor serum potassium periodically and replace potassium as needed. The irbesartan component may partly offset losses but does not remove the need for monitoring.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
