Summary
Beta-2 agonists such as pirbuterol can lower serum potassium, and this adds to the potassium loss from hydrochlorothiazide. Hypokalemia may predispose to arrhythmias, particularly with higher bronchodilator doses or in patients with heart disease.
What this interaction means in plain words
Hydrochlorothiazide-irbesartan and Pirbuterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Beta-2 stimulation drives potassium into cells while thiazides increase renal potassium excretion, producing additive hypokalemia.
What to do
Monitor serum potassium and ECG in at-risk patients, especially with frequent or high-dose pirbuterol use. Replace potassium if levels fall.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
