Summary
Xipamide is a thiazide-like diuretic and adds to the diuretic and antihypertensive effects of the hydrochlorothiazide component, while irbesartan adds further blood pressure lowering. The combination increases the risk of hypotension, volume depletion, hyponatremia, hypokalemia and renal function decline. Duplicating thiazide-type therapy offers little benefit.
What this interaction means in plain words
Hydrochlorothiazide-irbesartan and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive inhibition of distal tubular sodium reabsorption and additive blood pressure lowering.
What to do
Avoid duplicate thiazide-type diuretic therapy unless deliberately intended, such as sequential nephron blockade in resistant edema. Monitor electrolytes, renal function and blood pressure.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
