Summary
Ibuprofen can reduce the antihypertensive effect of the thiazide and ACE inhibitor. Combined with a diuretic and ACE inhibitor it increases the risk of acute kidney injury and hyperkalemia, especially in dehydrated, elderly or renally impaired patients.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Ibuprofen can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Hydrochlorothiazide-lisinopril belongs to Cardiovascular; Ibuprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing the diuretic and ACE inhibitor.
What to do
Avoid regular use where possible and prefer paracetamol. If an NSAID is needed, use the lowest dose briefly and monitor blood pressure, creatinine and potassium.
