Naproxen can blunt the antihypertensive effect of the ACE inhibitor and thiazide and, in combination with them, increases the risk of acute kidney injury and hyperkalemia, especially in older or volume-depleted patients.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Naproxen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-lisinopril belongs to Cardiovascular; Naproxen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis causes sodium and water retention and reduced glomerular perfusion, which opposes the antihypertensive and diuretic effects.
What to do
Monitor blood pressure, serum creatinine, and potassium, and use the lowest NSAID dose for the shortest time. Maintain hydration and consider paracetamol where suitable.
What to ask your pharmacist
