Summary
Sulindac can reduce the blood pressure lowering effect of losartan and hydrochlorothiazide. It also adds to the risk of acute kidney injury and hyperkalemia, though sulindac may have less renal prostaglandin effect than other NSAIDs.
What this interaction means in plain words
Hydrochlorothiazide-losartan and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-losartan belongs to Cardiovascular; Sulindac belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing ARB and diuretic effects.
What to do
Monitor blood pressure, serum creatinine and potassium, and keep patients hydrated. Use the lowest effective dose for the shortest duration.
