Summary
NSAIDs such as tolfenamic acid can blunt the antihypertensive effect of losartan and hydrochlorothiazide. In patients who are older, volume-depleted or have renal impairment, the combination can cause acute kidney injury and raise potassium.
What this interaction means in plain words
Hydrochlorothiazide-losartan and Tolfenamic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-losartan belongs to Cardiovascular; Tolfenamic acid belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis causes sodium and water retention and reduces glomerular perfusion, which is dependent on prostaglandins when the renin-angiotensin system and diuretics are acting.
What to do
Use the shortest NSAID course at the lowest dose and ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium, particularly in the first weeks and in high-risk patients.
What to ask your pharmacist
