Summary
Revefenacin is converted to an active metabolite that is a substrate of OATP1B1 and OATP1B3. Inhibition of these transporters by telmisartan may increase systemic exposure to the metabolite and anticholinergic effects such as dry mouth, urinary retention and tachycardia.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Revefenacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-telmisartan belongs to Cardiovascular; Revefenacin belongs to Respiratory.
Why it happens
Inhibition of OATP1B1/1B3-mediated hepatic uptake of revefenacin's active metabolite.
What to do
Monitor for anticholinergic adverse effects when combined. Consider an alternative inhaled bronchodilator if effects are troublesome.
What to ask your pharmacist
