Summary

NSAIDs can blunt the antihypertensive effect of ARBs and thiazides and reduce renal perfusion. In older or volume-depleted patients, this triple effect can cause acute kidney injury and hyperkalemia.

What this interaction means in plain words

Hydrochlorothiazide-valsartan and Mefenamic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hydrochlorothiazide-valsartan belongs to Cardiovascular; Mefenamic acid belongs to Musculoskeletal.

Evidence level: High

Why it happens

Inhibition of renal prostaglandin synthesis causes sodium and water retention and reduced glomerular perfusion, which counteracts the antihypertensive effect and impairs renal function when combined with ARB and diuretic.

What to do

Use the lowest effective NSAID dose for the shortest time and ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium, especially in the elderly or those with renal impairment.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
