Summary

Iloperidone is a weak time-dependent CYP3A4 inhibitor and may modestly raise systemic salmeterol exposure. Higher salmeterol levels can increase tachycardia, tremor and QT effects. Iloperidone itself also prolongs the QT interval, so additive cardiac risk is possible.

What this interaction means in plain words

Iloperidone and Salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Iloperidone belongs to Central Nervous System; Salmeterol belongs to Respiratory.

Evidence level: Low

Why it happens

Iloperidone inhibits CYP3A4-mediated metabolism of salmeterol, with possible additive QT prolongation.

What to do

Monitor heart rate, palpitations and ECG in patients with cardiac risk factors. Reduce salmeterol use if beta-agonist adverse effects appear.
