Summary

Tipranavir given with ritonavir may increase imipramine concentrations, raising the risk of anticholinergic effects, sedation, arrhythmia and seizures. Ritonavir-boosted regimens inhibit several CYP enzymes involved in tricyclic metabolism.

What this interaction means in plain words

Imipramine and Tipranavir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Imipramine belongs to Central Nervous System; Tipranavir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Inhibition of CYP2D6 (and other CYP pathways) by the tipranavir/ritonavir combination reduces imipramine clearance.

What to do

Consider starting imipramine at a lower dose and titrate by response. Monitor ECG and tricyclic levels if available.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
