Nelfinavir inhibits CYP3A4 and P-glycoprotein, which can raise systemic exposure to indacaterol. Higher exposure may increase beta-agonist effects such as tachycardia, tremor and hypokalemia. The effect is modest at the usual inhaled dose.
What this interaction means in plain words
Indacaterol-glycopyrronium and Nelfinavir have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Why it happens
Inhibition of CYP3A4 and P-glycoprotein by nelfinavir reduces indacaterol clearance.
What to do
No dose change is generally needed at the standard dose. Monitor heart rate, tremor and potassium if symptoms of beta-agonist excess appear.
