Summary
Ranolazine prolongs the QT interval in a dose-related manner, and indapamide can add to this through its own QT effect and through electrolyte depletion. The combination increases the chance of torsades de pointes, especially when potassium or magnesium is low.
What this interaction means in plain words
Indapamide and Ranolazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Indapamide belongs to Cardiovascular; Ranolazine belongs to Cardiovascular.
Why it happens
Additive QT prolongation, with diuretic-induced hypokalemia and hypomagnesemia increasing arrhythmia susceptibility.
What to do
Correct and monitor potassium and magnesium and obtain a baseline and follow-up ECG. Consider an alternative diuretic or antianginal if the QTc becomes prolonged.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
