Summary
The intranasal influenza vaccine contains live attenuated virus, which can replicate in a patient whose immune system is suppressed by B-cell depletion. This creates a risk of vaccine-strain infection and a poor immune response.
What this interaction means in plain words
Influenza virus vaccine quadrivalent intranasal and Ocrelizumab are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Ocrelizumab-induced immunosuppression allows uncontrolled replication of live attenuated vaccine virus and impairs vaccine response.
What to do
Do not give live intranasal influenza vaccine during ocrelizumab therapy or until B-cell repletion; use an inactivated injectable vaccine instead.
