Summary
Miglitol slows carbohydrate digestion and lowers postprandial glucose, which adds to the glucose-lowering effect of insulin. Hypoglycemia may occur more often or more severely when the two are combined. Because miglitol delays absorption of sucrose, oral sucrose is ineffective for treating the hypoglycemia.
What this interaction means in plain words
Insulin degludec / insulin aspart and Miglitol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive pharmacodynamic glucose lowering from alpha-glucosidase inhibition plus insulin action.
What to do
Monitor blood glucose closely and reduce the insulin dose if hypoglycemia occurs. Treat hypoglycemia with oral glucose (dextrose) or parenteral glucagon rather than sucrose.
