Summary
Miglitol slows carbohydrate absorption and lowers postprandial glucose, which can add to insulin's effect and cause hypoglycemia. Treating hypoglycemia with sucrose is less effective while on miglitol, so oral glucose must be used.
What this interaction means in plain words
Insulin lispro and Miglitol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Insulin lispro belongs to Endocrine; Miglitol belongs to Endocrine.
Why it happens
Alpha-glucosidase inhibition delays carbohydrate digestion, adding to the glucose-lowering effect of insulin.
What to do
Monitor glucose and adjust the insulin dose when starting miglitol. Advise patients to treat hypoglycemia with glucose (dextrose) rather than table sugar.
