Summary
Neostigmine boosts acetylcholine activity while ipratropium blocks muscarinic receptors, so the two have opposing pharmacology. With inhaled ipratropium at usual doses, systemic exposure is small and a meaningful clinical effect is unlikely. Antimuscarinic effects of ipratropium may partly blunt the bronchial and cholinergic effects of neostigmine.
What this interaction means in plain words
Ipratropium and Neostigmine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Ipratropium belongs to Respiratory; Neostigmine belongs to Central Nervous System.
Why it happens
Pharmacodynamic antagonism between a cholinesterase inhibitor and a muscarinic antagonist.
What to do
No routine change is needed; watch for bronchospasm or excess secretions in patients on neostigmine who also use ipratropium, and adjust bronchodilator therapy if needed.
